You can request an account deletion via account settings.

It goes up because someone had received a refund for what they paid for your Custom Content. You could have the items restored and anyone who hasn’t already refunded it will not be offered one since the items would still exist on their CC.

Make sure you haven’t disabled servers adding files to your game in the Gmod multiplayer settings.

There is no option to revoke permissions.

Players cannot revoke access to cc items on their own once it's given to you or purchased via the CC editor. Only if they go inactive. You would lose access until they come back. You are correct though that if you are "gifted" permissions, you do not get a refund. Also if someone else purchases the weapon/model after they go inactive you would lose access to it until the new owner goes inactive and if/when it's restored by the guy that gave it to you.
