Blue made great points, and stating my reasoning for these would basically just be quoting Blue's reply, but to keep it short; "the trial period is a learning experience."

nah the dude who killed rayen wasnt on any doors or anything - that wasnt their base they just threw their boxes in it.

Usually you'd want to put the reason for the FailRP in the warn. Adding a note apart from the warning would make it seem like 2 different situations happened and it could lead to confusion among staff.
